The entrapment of the dorsal scapular nerve may cause neck, shoulder and interscapular pain.
The diaganosis of this neuropathy is a challenge because these symptoms are usually considered as a cervical spine manifestation. A characteristic feature of this disease is the appearance of pulling pain along the medial edge of the scapula. In the advanced cases winging scapula may occur.
Anatomy:
The dorsal scapular nerve arises from the brachial plexus.The nerve pierces the middle scalene muscle and then it passes under the levator scapula muscle to innervate the rhomboid muscles. The course of the nerve can be tracked during ultrasound examination.
Symptoms that may indicate the dorsal scapular nerve neuropathy:
- shoulder pain of unknown etiology,
- results of radiologic examination of the shoulder joint,
- pain radiating along the medial edge of the scapula,
Treatment:
In the absence of improvement after conservative treatment, a diagnostic block with local anesthetic should be performed to confirm diagnosis. The treatment is the dorsal scapular nerve hydrodissection.
