The superior cluneal nerves are derived from the cutaneus branches of the dorsal rami of L1,L2,L3 and innervate the skin of the upper part of the buttocks.

Some of the nerve fibres pass through the osteofibrous tunnel created by the illiac crest and thoracolumbar fascia. It is a common site of a nerve compression.

Symptoms of superior cluneal nerves entrapment:

  • neuropathic pain of illiac crest area,
  • neuropathic pain radiating to upper buttock area.

Sometimes correct diagnosis of lumbar pain is a real challenge. One of the causes of lumbar pain can be a cluneal nerves entrapment. To confirm a diagnosis it is necessary to perform a diagnostic block. In the case of resolution of symptoms it is reccomended to perform a cluneal nerves hydrodissection.
